Back in 2012, I blogged twice about aspects of Dr. Donald M. Berwick's 1989 article in the New England Journal of Medicine titled “Continuous Improvement as an Ideal in Health Care.” The full text is only available to subscribers.

Honda is recalling 1.2 million Accord cars from the 2013-16 model years in the United States over a battery sensor issue, according to several media reports. The recall addresses the potential for an engine compartment fire, reports the New York Times . Honda reportedly says that the sensors on the negative terminal of the battery are not properly sealed from moisture; as a result, road salt can get in and cause corrosion and an electrical short

ASTM A967/A967M Updated – Chemical Passivation

ASTM A967/A967M has just been updated with the release of the new 2017 Edition.  This specification is a best seller here at Document Center Inc.  The title is Standard Specification for Chemical Passivation Treatments for Stainless Steel Parts.  This new revision replaces the previous 2013 Edition, which is now obsolete.  (For more information on the old edition, please see my post from 2013:  New ASTM A967/A967M 2013 Edition).

Fiat Chrysler Automobiles NV said on Friday it is recalling 1.33 million vehicles worldwide in two separate campaigns for potential fire risks and inadvertent airbag deployments. The Italian-American automaker said it is recalling about 770,000 sport utility vehicles because of a wiring issue that may lead to inadvertent deployment of the driver-side air bag and is linked to reports of five related minor injuries, but no crashes. Honda U.S. to Recall Odyssey Vehicles to Inspect and Replace Seatbelts

Honda will voluntarily recall approximately 3,900 Odyssey vehicles from the 2018 model year in the United States to inspect and, if needed, replace the third-row center seatbelt, free of charge.

Third-row seatbelts use different seatbelt tongues for the outer seating positions and the center seating position. During third-row center seatbelt assembly at a supplier, outer seatbelt tongues were mistakenly installed on to center seatbelt assemblies. AMS 2404 Electroless Nickel Plating Updated

AMS 2404, Plating, Electroless Nickel, has been revised.  The new Revision H is available now from Document Center Inc. in both paper format and for pdf download.  The standard was originally issued in 1957 and is a mainstay of the plating industry.  The new Revision H results from the usual SAE 5 year review process.  It has a number of changes you'll need if you use this specification.

Medication Errors Resulting in Negative Outcomes on the Rise

A new study reveals that medication errors outside of medical facilities resulting in serious medical outcomes is on the rise in the United States.

The study, conducted at the Center for Injury Research and Policy and the Central Ohio Poison Center at Nationwide Children’s Hospital examined calls to Poison Control Centers across the country over a 13-year period.

Researchers looked at exposures to medication errors that occurred primarily in the home and resulted in serious medical outcomes.

German Carmaker Daimler Facing its Own Emissions Scandal

Daimler is alleged to have sold over a million cars in Europe and the United States containing engines rigged to cheat emissions tests. German authorities raided several locations associated with the automaker back in May.

EN 13200-8 is the latest addition to the EN 13200 series for sports venues.  Titled  Spectator facilities - Part 8: Safety Management, it focuses on safety management for these types of facilities.  As a European standard, it has direct application to the continent's sports facilities.  However, it can certainly be used anywhere with good results.

Primewell Tires Recalled; Sidewall Can Crack and Lose Air

A company that distributes Primewell brand tires is recalling more than 394,000 of them in the United States because the sidewalls can crack and lose air, increasing the risk of a crash.

The recall covers certain Valera Touring II replacement passenger car tires distributed by California-based Giti Tire USA and sold at Firestone stores.

There's a new ISO 18295 series for customer contact centers.  These 2 publications reflect the increase in "service standards."  The goals of such standards are to define expectations, provide guidelines for performance management and support customer satisfaction.  The customer contact centers addressed by the ISO 18295 Parts 1 and 2 may be in-house or outsourced.  The specific responsibilities of the centers can vary.
